Privacy notice

The request form

We collect your email address, your first name if you give it, the NHS area, where you are in the process, and the date of the decision letter if you give it. We use these only to reply to you about your review, and to tell you when free reviews open. Our lawful basis is your consent, which you can withdraw at any time by emailing us.

The form does not ask for names or medical details, and we ask you not to include them.

Documents you send later

If you go ahead with a review, we will send a secure way to share the decision letter and the assessment form. These contain health information, which the law treats as special category data. We will ask for your explicit consent first, and confirm that you have the authority to share them (for example a lasting power of attorney). We use them only to prepare your review and delete them within 30 days of sending it, unless you ask us to keep them.

Page counts

We count how many times each page is viewed, and which website sent the visitor. We do not use cookies, do not store IP addresses, and do not use any third-party tracking.

Where it is kept and for how long

Information is stored with Amazon Web Services in London. Request-form details are deleted 12 months after we last hear from you. The score checker and deadline calculator run in your browser and send nothing to us.

Your rights

You can ask to see, correct or delete your information, or object to how we use it. Email us and we will reply within one month. If you are unhappy, you can complain to the Information Commissioner's Office.
