Continuing healthcare appeal letter template
A plain template for asking the NHS to review a continuing healthcare decision, called "local resolution". Copy it, fill in the parts in square brackets, and keep a copy of everything you send.
Short of time? If your deadline is close, send only the first two paragraphs now to say you are appealing, and send the evidence later. Most NHS areas allow 6 months from the date on the decision letter. Work out your date.

I am writing on behalf of [name], as their [relationship, and lasting power of attorney for health and welfare if you hold one]. I ask you to review the decision that [name] is not eligible for NHS continuing healthcare, under your local resolution procedure.
Please send me the full continuing healthcare file, including the completed Decision Support Tool, the multidisciplinary team's recommendation, the reasons for the decision, and the records relied on, if you have not already done so.
I believe the following areas of care were scored too low:
[Area of care, for example Behaviour]. Level given: [High]. Level the evidence
supports: [Severe].
[Two or three dated examples from the care records, with where they are, for example: "On 3 June
the daily notes (page 14) record that two staff were needed to prevent harm during personal
care."] [One sentence on how these match the wording of the higher level in the assessment
form.]

With these levels, [name] would have [for example: two severe needs], which the national guidance says would be expected to lead to a clear recommendation of eligibility. Where needs are well managed, they are still needs, as the guidance states.

Tips
- Facts beat arguments. Dated examples from the records carry more weight than general statements or legal references.
- Use their words. Match your evidence to the wording of each level in the assessment form.
- Only argue the areas that matter. Use the score checker to see which changes would reach the national threshold.
- Keep copies and note the date you sent it.
